The AF4Q Evaluation uses a mixed-methods approach to provide inferences about the impact of AF4Q, identify lessons learned, and expand the knowledge base around the effectiveness of the multi-stakeholder approach to improving health care at the community level.

Alliance Survey
The Alliance Survey is administered regularly over the course of the AF4Q program by the AF4Q Evaluation to understand alliance member perspectives on alliance operations, leadership, decision-making, etc.
Consumer Survey
Two rounds of the Consumer Survey have been administered with a random sample of chronically ill individuals across the U.S. This survey focuses on health care consumer’s experiences receiving care, awareness and use of health care quality information, and levels of consumer knowledge and confidence in managing their own health and health care (patient activation).
Key Stakeholder Interviews
The AF4Q Evaluation has conducted interviews with a variety of stakeholders over the course of the AF4Q program. These open ended interviews are designed to collect in-depth information about how the AF4Q program is implemented, the context within which the program operates, and multiple opinions and perspectives on the overall value of AF4Q.
National Survey of Small and Medium-Sized Physician Practices (Physician Survey)
Two rounds of the physician survey have been administered with physicians in small and medium sized practices in AF4Q communities and across the nation. The purpose of this survey is to understand the types of quality improvement activities being utilized by physician practices.
